Sailor Moon

You'll start the final chapter with a Planisphere for a weapon and the Wand to transform into Usagi (Serena). You'll immediately see Luna outside the town, who'll promptly steal your Gold Gems and take off. Oops, I guess that was an imposter. But then the real Luna will appear and join up with you and tell you to meet up with Artemis at the Crown Game Center. One thing I should mention about this chapter is that there is a slight modification to the battles. If you get three stars, instead of the usual "Special Attack", Tuxedo Mask will pop out and throw roses at your opponent. Just thought I'd mention it, since you may or may not be lucky enough to actually see it happen.
As you might expect, as the final chapter in the game, this city would have some great amenities, and you'd be right. Feel free to upgrade your equipment if you'd like. In the northeast corner of town, you can find some invisible money in front of this building, so pick it up.
If you check out the northwest corner of town, you can find the God of Happiness, who will give you 500 more Peach.
Now, time to meet up with Artemis. Crown Game Center can be found in the southwest corner of town. Here you will find Motoki, who is apparently possessed. You'll need to defeat him to proceed. Get used to this, as you'll have to do this to almost all the Sailor Moon characters you run into in this chapter.
After defeating Motoki, he'll tell you to move aside the conspicuous arcade machine. Do so and you'll find Artemis, who gives you his phone number and tells you to gather all the Sailor Soldiers (Sailor Scouts) together. If you talk to Motoki again, you'll receive a tip to check up Crystal Seminar Cram School.
Crystal Seminar Cram School is in the southeast corner of town. On the second floor, you'll run into a chap claiming to Umino (Melvin), despite his lack of glasses. I'm quite sure the programmers goofed here and switched his sprite with the random NPC downstairs. In any case, you'll have to defeat him to move on.
You will find Ami (Amy) past Umino. She'll (unexplicably) have brown hair in the original Japanese release, but her hair should look a little bluer in the translation. In either case, talk to her and she'll join you and tell you to head to the park next. You'll notice after she joins up with you that the Sailor Mercury symbol will be added to your reels in battle. If you land on them, she will appear and perform one of two powerful attacks. As you gather the Sailor Soldiers, you'll get more and more of their symbols, making battles a bit easier.
The park is in the southern portion of town. Float over the fence by talking to the clown, then enter the house. In it, you'll find a Cap Gun in the chest and, more importantly, Rei (Raye). Join up with her and she'll tell you to check out the bridal shop next.
The bridal shop is just northeast of the park. Upstairs, you will run into Naru, who, like everybody else, is possessed. Beat some sense into her and she'll exchange your shabby old Crystal for a Diamond.
Just past Naru is Makoto and a chest containing 1000 Peach. After Makoto joins you, she will give a bus pass and tell you to head to Tokyo Tower. So go and do so.
It's also worth noting that if you talk to Naru at any time from this point on, she will inexplicably restore your HP and MP. I don't know why this happens, but it beats having to spend your funds at the hospital!
Go south of the southeast corner of town to reach the bus stop. Take a ride to Tokyo Tower and try to enter. You'll find that the door inside is locked. By talking to this guy, you'll discover that a black cat has the key. Hmmmm...
Just outside Tokyo Tower is the imposter Luna. Take care of her and you'll get your Gold Gems back, along with the key to Tokyo Tower.
Unlock the door in Tokyo Tower and head up the stairs to find Minako. She'll tell you that you need to return to the city to find Usagi (Serena).
Unfortunately, though, you have no bus pass. However, just talk to the guy at the tower's entrance and he'll hook you up with one.
You can find Usagi in the northwest corner of town, in an arcade that was previously unaccessable. Once you rescue her, she'll remind you that you need to visit Artemis again.
In case you couldn't remember, Artemis is found in the arcade in the southwest corner of town. When you talk to him, he'll give you a Transformation Brooch. If you're playing in Usagi-form, this will transform you into Sailor Moon in battle. If you're in your regular form, you'll be transformed into a Sailor Soldier version of yourself. Either way, while it looks pretty cool, it doesn't seem to make you any more powerful.
Artemis also gives you a bus pass telling you to return to Tokyo Tower one last time for a final showdown with Rakira. You can do this any time you'd like, but once you leave, you can't go back, so make sure to wrap up any loose ends before you go. There's also one small sidequest you can embark on before you go. Head up to the northeast corner of town, you'll find that the building near the invisible 500 Peach you picked up earlier can now be entered. It looks like an empty Jewelry Shop on the inside. Head to the back to find a chest with 1000 Peach, and a shady looking character named Kaseet. Kaseet, along with Waruji, is one of Rakira's henchmen, and as such, you cannot run from him. You also can't use Ice Magic on him. But you CAN defeat him, so go ahead and do so.
After defeating Kaseet, walk to the back and talk to the lady there. It's apparently the Jewelry Shop owner. After rescuing her, this shop will be opened, allowing you to buy a Garnet, which is not useful at this point in the game, and a Lapis, which is the most powerful gem in the game. You should be able to afford a Lapis, since you got 2000 Peach from the Kaseet sidequest, but at this point in the game, it's ultimately unecessary. In any case, get one if you want, then head back to Tokyo Tower.
In Tokyo Tower, you'll notice that this door leading to the basement is now mysteriously unlocked. So walk this way.
The first enemy you encounter after going down the first flight of stairs is somewhat special. Unlike the other enemies in the tower, this one respawns. And while the other enemies drop Peach, which can't be used in the tower, this one drops Cookies, which is the most powerful healing item in the game. I recommend defeating it over and over to fully stock up on Cookies before continuing on the tower. Besides, this is the only way to reliably restore your HP/MP at this point in the game anyway.
Tokyo Tower is mind-numbingly linear, so just keep on keeping on until you run into Rakira. Rakira's similar to Kaseet in that you can't run from him in battle, but unlike all the other enemies in the game, absolutely no magic works against him. But you'll probably be landing on so many Sailor Scouts and the like that you probably won't even notice.
After you finish off Rakira, he'll just poof away (rather quickly for a final boss) and you will find the final Gold Gem. The game is now over. A winner is you.
